- Bring the following to your appointment:
- Updated medication list (Bring your medicines if necessary because we want to make sure this is accurate and NOT depend on what is on your electronic medical record at another office)
- The actual bottle or tube of medicine(s) into the office for any skin conditions you are already treating
- Insurance card
- ID
- Payment method (cash, credit card, check) for your copay or unmet deductible
- Print and complete new patient paperwork
- Some exams are focal based on your concerns (for example, an acne exam usually involves the face) but sometimes a full skin exam is recommended. This involves removing all clothing except underwear and being covered in a gown that we provide. But, we can limit the exam based on your preferences.
- Arrive 10 minutes early for new patient paperwork
